Acne and Rosacea Pearls

Aspen, Colo. – At this summer’s edition of The Cosmetic Bootcamp (CBC), San Antonio-based dermatologist Vivian Bucay, M.D., shared tips for treating acne and rosacea:

  • Set goals – “When patients come to the office, no matter what for, I always ask, ‘What do you want to accomplish in this visit?’ This gives them the power to express themselves and their expectations. You can decide whether that’s something you can do for them. It’s a contract.”
  • Sometimes it’s just redness – Patients may say, ‘I have this terrible rosacea.’ And all they really have is long-standing UVB damage.” UVB advances expression of vascular endothelial growth factor (VEGF), she said. For these patients, “You may still use an energy device. But it may not be true rosacea, with the implications of the blushing, flushing and triggers.”
  • Choose aesthetic treatments carefully – Consider the patient’s lifestyle and habits (no smokers!). “If you’re going to use fillers, does the patient have hypertrophic, boxcar or icepick scars? I’ve become a fan of the chemical reconstruction of skin scars (CROSS) technique using 100% TCA, applied focally. It can be very effective.”
