Registration & Fees
New York Covid Requirements
Proof of vaccination (at least one dose) will be required for everyone upon check-in at the Harvard Club of New York City.
Proof of vaccination may include:
– CDC Vaccination Card(or photo)
– An official immunization record from outside NYC or the U.S
If you received the vaccine outside the U.S., you must have an official immunization record that includes:
– First name and last name
– Date of birth
– Vaccine product name (only vaccines authorized by the WHO are acceptable)
– Date(s) administered
– Site where the vaccine was administered, or name of the person who administered it
Since vaccines have not been approved for children under 12, those individuals may enter the Clubhouse so long as they wear a face mask, except when eating and drinking.

Diane S. Berson, MD, FAAD
Diane S. Berson is an Associate Professor of Dermatology at Weill Medical College of Cornell University and an Assistant Attending Dermatologist at the New York-Presbyterian Hospital. She has a private dermatology practice in NYC focused on medical, surgical, and cosmetic dermatology and cutaneous oncology.
Jeanine B. Downie, MD
Jeanine B. Downie, M.D. is board certified by the American Board of Dermatology and holds medical licenses in the states of New Jersey, New York, and California. Dr. Downie is the director of her own practice, image Dermatology P.C. in Montclair, N.J. She has extensive training and experience in all aspects of dermatology, cosmetic dermatology, laser, and dermatologic surgery.
José Raúl Montes, MD, FACS, FACCS
Dr. Montes is an established master injector being recognized among the nations’ top injectors. In 2016, he received an Achievement Award as well as a Continuing Education Recognition from the American Academy of Ophthalmology. As a Professor of the University of Puerto Rico School of Medicine, Dr. Montes has been mentoring students at the Río Piedras Medical Center.
